Wellness Membership

Your membership subscription is charged on the same date every month to your credit card on file.  Your fee entitles you to the massage benefits that are listed for the membership of your choice.  Massage benefits cannot be refunded.  Missed massage can be made up at an time while the contract is valid.  Massage benefits expire 60 days after your contract is cancelled.  

It is always best to schedule the next massage when you are in office.  You can also book online, text or call us.  We will work with you to find a convenient time.   Please be courteous and give at least 24 hrs notice to cancel or reschedule any treatment.  Late Cancellations or No shows will be charged a $25 cancellation fee to the card on file.  

After 90 days you may cancel at any time with 30 days written notice.  Any unused massages can be used up to 30 days after contract is terminated with proper notice..

  It is your responsibility to inform your therapist or esthetician of any pre-existing conditions, limitations or specific sensitivities and to inform you provider if you feel any discomfort during the session. If you do experience discomfort, please communicate it to your provider and ask him/her to adjust accordingly. You understand and voluntarily accept any risks of which you have been advised associated with your treatment, or form any use of the companies facilities, and hereby release Holistic Health Therapies (including its employees, practitioners, agents, and insurers) from all liability for any injury, including, without limitation, personal bodily or mental injury, economic loss, or damage to you resulting there from. You further hereby release all of the foregoing personnel and entities from all liability arising from any such injury or damage resulting from your failure to disclose any pre-existing condition, limitation, or specific sensitivities, or your failure to inform your provider of any discomfort during your session. Your therapist or esthetician may determine that it is unsafe for you to proceed with or continue a therapeutic session due to health related concerns. In this event you may be required to provide Holistic Health Therapie with a physician’s medical release prior to continuing treatment.
